I am one of the first EM trained docs 39 years ago,,practice in 2 level one and work 17 shifts/month Love the personality of EM. when i started was part of a democratic group which in reality was amorphous (work get $, dont work move on). Then mega groups came along and convinced the admin from Washington to the C-suites they could meet metrics, dump bad docs and “oh yall make sure they are good clinicians”. It was so satisfying to get buy in from other specialties about our cause and it wasn”door to doc, decision to admit, and did they get their antibiotics before they got out of the car so we could give them some good old c.diff.
    The new generation EM residents are so misguided about the business of EM, it diminishes their academic pursuits. The mega group puts in medical directors who drank the cool-aid and protect that contract.. We are dying until we shake the “contract holders” and can deliver great care (we all become a patient at some time). Pay us for being up at 3 am and lets have brisk CQI and grow not be a spot on spread sheet cant sign with real name as I will be the blank on spread sheet looking for another job!!! WOW we blew it as we were so needed instead of just moonlighting guys from every specialty REMEMBER how SCARY!!
